The project “A path through life more beautiful thanks to activity and social integration”, within which the Family Care Home Sp. z o. o., using funding from European Union funds, implements the “Respite care” program, is addressed to caregivers of people with a moderate or significant degree of disability, living in a common household in the following municipalities: Pierzchnica, Morawica, Chęciny, Raków, Chmielnik, Daleszyce, Łagów, Staszów, Gnojno, Busko-Zdrój, Sobków, Jędrzejów.

Family Care Home Sp. z o. o. provides care for a person who needs support in everyday functioning as a replacement for the actual caregiver for a period not longer than 60 days a year, providing 24-hour nursing and care, full board, occupational therapy and rehabilitation adapted to the health condition of the disabled person and their individual requirements. In addition, we will provide support to informal/actual caregivers in the form of training in the field of care and care for a disabled person, first aid training, legal and psychological consultations, anti-discrimination workshops.

The list below contains all documents that must be completed, signed and delivered, along with a photocopy of the disability certificate of the person requiring support in everyday functioning, to the PROJECT SERVICE OFFICE located at ul. Kardynała Stefana Wyszyńskiego 2 26-015 Pierzchnica, in order to participate in the recruitment process of project participants. It is possible to send documents to the headquarters of the PROJECT SERVICE OFFICE by post/courier.

How to complete the documents?

Below we have included detailed instructions to help you fill out each document correctly. We encourage you to read these guidelines carefully before you start filling out the forms.

STEP 1

The actual/informal guardian of a disabled person requiring support in everyday functioning is obliged to complete the application form dedicated to the guardian in the scope of:

section no. I regarding PERSONAL DATA (only in sub-item A)
section no. II by selecting the answer consistent with the actual state of affairs
section no. III by selecting sub-item no. 2 regarding “Respite care”
The completed form should be signed legibly and the place and date of completion should be entered.

STEP 2

The actual/informal guardian of a disabled person requiring support in everyday functioning is obliged to sign two information clauses regarding the processing of personal data by the Beneficiary and the Managing Authority.

STEP 3

A disabled person who requires support in everyday functioning is obliged to complete the application form dedicated to disabled persons in the scope of:

section no. I concerning PERSONAL DATA. In the event that the disabled person has been incapacitated, in subsection B, enter the data of the legal guardian of the disabled person.
section no. II by selecting the answer consistent with the actual state of affairs
section no. III by selecting subsection no. 2 concerning “Respite care”
The completed form must be signed legibly and the place and date of its completion must be entered. In the event that the disabled person is incapacitated, only the legal guardian signs the form.

STEP 4

A disabled person who requires support in everyday functioning is required to sign two information clauses regarding the processing of personal data by the Beneficiary and the Managing Institution.

In the event that the disabled person is a legally incapacitated person, the clauses are signed only by the legal guardian.

STEP 5

Frequently Asked Questions (FAQ) Section:

How should I prepare and submit project application documents?

All required documents must be downloaded in PDF format from the website, printed, filled in by hand and signed. The complete set of documents can be submitted only in paper form – in person at the Project Service Office or sent by traditional mail. It is not possible to submit documents by e-mail or in electronic form.

What documents are required to be submitted?

The required documents are: Application form for the guardian, Application form for the person requiring support/disabled, 2 x Information clause signed by the guardian (Beneficiary and Managing Institution), 2 x Information clause signed by the person requiring support/disabled (Beneficiary and Managing Institution), photocopy of the disability certificate of the person requiring support/disabled.

Do I have to submit all documents at once?

YES

Who are we recruiting?

We are currently recruiting for 23 men and 36 women requiring support in everyday functioning with a moderate or significant degree of disability.

When is it possible to be accepted into the project?

The expected start date of the next project cycle is September 2025.

How will I be informed about qualification for the project?

If you qualify for the project, we will contact you by phone.

Who CANNOT be included in the project?

Family Care Home Sp. z o. o. cannot provide nursing and care services to persons requiring 24-hour medical care (persons who are mechanically ventilated, fed by a tube, persons taking strong painkillers that can only be administered under medical supervision, persons who are mentally ill, or who abuse alcohol or narcotic substances).
